Based on the initial consultation we will present various senior living options, which may include assisted living communities, memory care facilities, nursing homes, independent living communities, residential care homes, or in-home care. 

Tours & Visits

We will schedule a tour at your convenience, allowing you to see firsthand the environment and services offered, accompanying you and your loved one to evaluate each option.

Decision

Once a decision is made, we coordinate a smooth transition handling paperwork and logistics.

Move Coordination

Our staff can help your loved one transition from home to the facility. They can assist with downsizing, de-cluttering, and even create an action plan that would best address any issues during the move.

Continued Follow-Up

After placement, our commitment does not end there, we stay in touch with families and seniors to ensure they are adjusting well and addressing any concerns that may arise.

Your family can schedule a tour at your convenience so that our care manager will show you homes in your area that meet your specific needs and budget.

Decision

We advise you of the pros and cons of each community, as well as their prices, so you can choose which senior housing option suits your family’s needs, wants, and budget.

Move Coordination
